Alliance Medical has created this privacy statement in order to demonstrate our firm commitment to privacy, in addition, we participate in the Council of Better Business Bureaus' privacy program. The following discloses our information gathering and dissemination practices for the following websites:

USE OF IP ADDRESSES:
We use your IP address to help diagnose problems with our server, and to administer our Web site. Your IP address is used to help identify you and your shopping cart and to gather broad demographic information. We use non-identifying and aggregate information to better design our website and to share with advertisers. For example, we may tell an advertiser that X number of individuals visited a certain area on our website, but we would not disclose anything that could be used to identify those individuals.

COOKIES:
Our site uses cookies to keep track of identities of our registered customers, to store our customers shopping cart items for future access by the customer. These cookies help to provide a faster service to you the customer. You can refuse cookies by turning them off in your browser. You do not need to have cookies turned on to use Alliance Medical's site. However, you do need cookies in order to place an order from our Online Ordering System, to access user's account information or to place an ad in our classifieds section. USER ACCOUNT REGISTRATION:
Our site's User registration form requires users to give us contact information (like your name, address, phone number and valid email address). Financial information (like your account or credit card numbers) will NOT be taken when a user registers. Users are not required to register to use our site, however they must be registered in order to use the "MyList" function of our Online Ordering System. We use customer contact information to identify the registrant. The customer's contact information is also used to contact the visitor about their Online Account when account privileges change. We do not share this information with outside parties except to the extent necessary to complete transactions for our users.

ORDER FORM INFORMATION:
Our site uses order forms for customers to request information, products, and services. We collect visitor's contact information (like your name, physical and billing address, phone number and email address) and financial information (like your account or credit card numbers). Contact information from the order form is used to send orders and information about our company to our customers. The customer's contact information is also used to get in touch with the visitor about their order if necessary. Financial information that is collected is used to bill the user for products and services. We do not store financial information on our servers or elsewhere in our company except for our records pertaining to that order only. We will not share this information with outside parties except to the extent necessary to complete that order.

Customers may order item to be shipped to another person or address than the billing address. Information used in the shipping section of our order form is used specifically for the processing of that order and will not be used for any other purpose than to complete the transaction of that order. The Shipping address for orders are only kept for our records pertaining to that order and are not used for any other purpose. We do not share this information with outside parties except to the extent necessary to complete that order.

ACCOUNT APPLICATION:
Our site allows Emergency Service Organizations and Health Care Facilities to apply for an Alliance Medical account online. These applications require the applicant to provide contact information (like company name, address, and phone number), company information (like stockholders or president) and financial information (like bank relationships). This information is used to qualify applicant for an account with Alliance Medical under our customer account terms. We process applications by verifying references or by using the service of Dun & Bradstreet. We do not share this information with outside parties except to the extent necessary to process that application.

CONTACT US FORM:
Our site makes use of forms to contact us by email. We collect visitor's contact information (like an email address) on this form. Contact information from this form is used to send response to questions and information about our company to our customers. The customer's contact information is also used to get in touch with the visitor in response to the reason the visitor contacted us. Users may opt-out of receiving future promotional mailings; see the choice/opt-out section below. We do not share these addresses with outside parties.

SURVEYS:
From time to time Alliance Medical may place surveys on their site. These surveys may ask visitors for contact information (like an email address). We use these surveys to gather user feedback to better improve our website. The customer's contact information is also used to get in touch with the visitor when something in their survey requires a response (i.e. a question in a comment field or to request more information about a suggestion made). Users may opt-out of receiving future promotional mailings; see the choice/opt-out section below. We do not share these addresses with outside parties.

CONTESTS:
We run contests on our site in which we ask visitors for contact information (like Name, address, and email address). We use contact data from our contests to send users information about our company. The customer's contact information is also used to contact the visitor in order to supply the award for the contest when they have won.. Users may opt-out of receiving future promotional mailings; see the choice/opt-out section below. We do not share these addresses with outside parties.

EXTERNAL LINKS:
This site contains links to other sites and Alliance Medical is not responsible for the privacy practices or the content of such Web sites.

PUBLIC FORUMS:
This site makes chat rooms, classifieds, forums, message boards, and/or news groups available to its users. Please remember that any information that is disclosed in these areas becomes public information and you should exercise caution when deciding to disclose your personal information.

Our Forums program allows you to enter a fake email address so that others will not know your real email address. This will also protect you from spamming parsers out there. Our program also allows anonymous postings, meaning you do not need to be registered in order to read postings or to post on our forums. Our company will not purposely collect personally identifiable information to sell or share with outside parties.

SECURITY:
We utilize the industry standard SSL Encryption when accepting your credit card information during our checkout process in the Online Ordering. To prevent unauthorized access, maintain data accuracy, and ensure the correct use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online. To protect your privacy and security, we also take reasonable steps to verify your identity before granting access or making corrections to customer accounts.

CHOICE/OPT-OUT:
Our site provides users the opportunity to opt-out of receiving future promotional communications from us at the point where we request information about the visitor. If there is not a Choice/opt-out option at the point where we request information than that information is not kept for use of promotional communications, it is only kept to complete the transactions that it was collected for, which is stated at the time of collection.

VIEWING/UPDATING CUSTOMER INFORMATION:
Registered Users:
Open-Billing Accounts: You may view your information by logging in and going to your User Area. This will show you the information that Alliance Medical has for your company/organization. Changes may be made in this area and then submitted. Submissions will go to our accounting department for approval. You will be notified and your account will be updated upon approval. You may also use the methods described for Non-registered Users.

Non-Open-Billing Accounts: You may view your information by logging in and going to your User Area. This will show you the information that Alliance Medical has in its records for you. Changes may be made in this area and upon submission will immediately update your account. You may also use the methods described for Non-registered Users.
